Company innovating ways to reduce healthcare costs.

I just came across this press release by a company called SynCare which says its services:

Risk assessments, telemonitoring, data management, home-visitation, and care coordination efforts along with patient incentives allow SynCare to promote patient self-management of chronic conditions by empowering them with education, knowledge, and resources.

These are precisely the type of services that can help with lowering the cost of healthcare for employers and government alike. What SynCare is providing is very similar what David Hom of Pitney Bowes spoke about at a breakfast seminar I attended in October. The video from the event is posted in the archives. Disease prevention and management takes a cooperative approach from all parties effected. This includes the employer, the health insurance company, the medical care provider, and the individual. SynCare's services seem to make this an efficient, simple process.
